What this is

This form starts a process. It does not schedule treatment, request an appointment, or commit you to anything.

What it does is let us begin a conversation and, if you choose to continue, organize your medical information so that an independently licensed physician can review your case and form an opinion.

That physician may conclude that this is not appropriate for you. If so, you will be told.


What happens after you submit

1. We send a short code to your phone to confirm the number is yours.

2. We begin a conversation by text message. You will be interacting with an AI-assisted system that helps organize information for physician review. You choose what to share and how quickly.

3. If you decide to continue, we explain which medical records are needed and help you obtain them.

4. A physician reviews your case.

5. If the physician determines that further discussion is warranted, you speak with them directly.

You can stop at any point. Reply STOP to any message and we will not contact you again.

What we are not asking for

We do not need, and will not ask for, your insurance information, Social Security number, or payment details.

We do not collect payment for healthcare services. Any fees are established and communicated directly by the treating physician, and you pay them directly.


If you are doing this for someone else

The patient needs to give their own permission, so this form should be completed by them or with them present.

If they are unable to do so and you hold legal authority to act on their behalf, submit the form and tell us during the conversation — we will explain what is needed.
